Dr. Rafalski serves as Associate Chief Medical Information Officer at Heart of Texas Community Health Center, in Waco, Texas. Dr. Rafalski is a family physician who has been in practice for 18 years.

A graduate of Johns Hopkins School of Medicine, he completed his Family Medicine residency at East Tennessee State University – Bristol. He is Board Certified in Family Medicine and Clinical Informatics, and is a Fellow of the American Academy of Family Physicians. His experiences include full spectrum family medicine from obstetrics to geriatric/nursing home care, group and solo practice, and teaching and lecturing. For 14 years he worked at Dayspring Family Health Center in the heart of rural Appalachia. He served as a physician, as well as Clinical IT director at Dayspring.

At the Heart of Texas Community Health Center, he is responsible for Quality Improvement, Workflow, Regulatory Reporting (MU, UDS, MIPS, ACO), and PCMH recertification. Matt is an EPIC certified Physician Builder. His IT interests include Patient – Physician relationship & communication while using EHR, Clinical Decision Support Strategies/Development, and QI/PI. He and his wife are the parents of 5 children and currently resides in Waco, TX.
