Building Bridges from Clinic to Community

Health problems, especially chronic diseases, usually require behavioral changes like nutrition and physical activity that cannot be solved in the exam room. This session will briefly describe the concepts of Community Oriented Primary Care (COPC) and Community-Centered Health Homes (CCHH), and describe how public health students have created systematic ways for patients to learn about community resources, empowering them to improve their health outcomes. The participants will engage in an interactive exercise to develop "bridges" from the clinic to the community in which they are working.
